Information About Registration Online

  1. Online Registration Available Hours

    • Morning practice: 07:30-10:00
    • Afternoon practice:14:00-17:00

    Registration cannot be made on prior days nor can it be made for the afternoon practice before 14:00

    If you neither could not register during the days hours listed above nor wish to register online, please come inside the hospital and register using reception device. The device can be used between 09:00-12:00 and 16:00-19:00.

    You cannot designate what time you would like to have consultation to start neither online nor using the reception device.

  2. Mandatory Information to Fill In (4 information)

    Please fill in four required information listed below

    • Family ID number (The number on the clinic card)
      If you have never been to the hospital and you don’t have Family ID, then please type “first time to come to the hospital”.
    • Pet owner’s last name
    • Pet name
    • Mobile phone number of who ever is coming to the hospital(Japanese phone number is preferable)

    ※If there are any errors or missing information, the conformation may be delayed and we may consult the next person in line ahead of you.
    To ensure a smooth process, we appreciate your cooperation in entering accurate information in advance.

  3. For the People Who Would Like to Designate Which Doctor You Would Like to Get Consultation

    If you would like to designate Doctor, please confirm your arrival at the reception before the hours listed below.

    • For morning practice: please confirm your arrival before 11:00
    • For afternoon practice: please confirm your arrival before 18:00

    If we cannot confirm your arrival by that time you might not be able to designate the doctor.

    Even if you do not which designate the doctor please confirm your arrival before hours listed below

    • For morning practice: please confirm your arrival before 12:00
    • For afternoon practice: please confirm your arrival before 19:00

    If we cannot confirm your arrival by that time, the consult will be canceled.

    PRECAUTION

    Please check the homepage to check if the English speaking doctor will present on the day you wish to have consultation.
    And English speaking doctor may not be present after 18:00 so we advice you to come to the hospital before 18:00

  4. When You Have Arrived at the Hospital

    After arriving at the hospital, please let the receptionist staff know that you have arrived.
    If the icon on the online waiting list has “Absent” mark beside the ID number, please come to the reception for the staff need to confirm that you have arrived at the hospital.
    Even if you are not first in line for the que online, please come to the reception so that the receptionist knows more about your pet’s purpose to the hospital and whereabout you’ll wait at(inside the hospital or at the parking space).

    We may hand you a beeper so that when you are inside the premises you’ll know when you can come to the examination room or when to do the payment at the reception.

  5. Paging Service

    When it is close to your consultation or other services, we will page you with paging service which starts with phone number 050-XXXX-XXXX. if you have registered Japanese cell phone for the waiting list online.
    If you have registered phone number that is not Japanese cell phone number, then we’ll let you know with using a beeper so please come to the reception and we will rent you the beeper.
    If you receive a paging service or beeper beeps, please come to the reception.

  6. If You Want to Temporary Leave the Hospital Premises

    After you’ve confirmed your arrival and if you want to temporary leave the hospital, please let the receptionist know (and return the beeper if we’ve handed you one) When you’ve returned to the hospital please let the staff know that you’ve returned. If we can’t confirm your return, then we may consult the patient after you.

  7. We May Correspond to Your Visit Faster Than Usual Consultation If It Is Listed Below

    • Boarding for Laboratory tests, surgery, for Hotel usage
    • Boarding out animals
    • Visiting boarded animals
    • If you’ve registered using waiting list online, please go to the receptionist to confirm your arrival.
  8. Appointment Cancellation

    We may cancel the appointment if you won’t be able to take call(s) or we can’t confirm your arrival.
    If you’ve registered using Line application, you can cancel your registration from the notice section inside the app.
